ACP Journal Club. Review: CURB65, CRB65, and Pneumonia Severity Index similarly predict mortality in community-acquired pneumonia

ABSTRACT

 

Dr. Morris comments on a study by Chalmers et al determining whether CURB65, CRB65 and Pneumonia Severity Index (PSI) differ for predicting mortality.  He contends that using severity assessment tools to identify patients who could be safely discharged from the emergency department is a process improvement that would save costs.  The PSI is the only risk assessment tool that has been validated for this purpose, and the review by Chalmers and colleagues suggests that it may have the best negative likelihood ratio for low-risk scores.  Clinicians may question whether the added complexity of the PSI is worth the benefit but, to this clinician, identifying additional patients who can be safely managed at home is certainly worth the effort.
